Participate in a confidential, ONLINE survey through Oregon State University about self-concept and health behaviors. Must be 18 years of age or older to participate.
1: Email or call with your height and weight.
2: If you meet the requirements, you will be sent a web address and an ID number for you to take the 15-minute survey. Paper copies are available too.
People that complete the survey will be entered into a drawing for a $50 gift card from Fred Meyer. 1 in 50 will win no matter how many participate, so please pass it on!
