Partnership for Health

The Ulman Cancer Fund for Young Adults is pleased to invite you to
be part of a new research project called Partnership for Health. It
was created by a team at Dana-Farber Cancer Institute, especially
for persons who have had cancer, leukemia, tumors or similar illnesses
as an adolescent or young adult. The goal of Partnership for Health is
to provide up-to-date health information about topics that are important to adults who have survived these illnesses. Resources for survivors
who smoke and want help quitting will also be available.

Here's how it works:

If you live in the United States or Canada and are interested in
Partnership for Health, please call 1-888-769-4745. You will speak
with a Partnership for Health representative and be asked a few
eligibility questions.

If you meet the criteria for enrollment, you will then be asked to
complete a 20-minute phone survey. The survey will include questions about your general health, your smoking habits, and your thoughts
and opinions as someone who has had cancer, leukemia, a tumor, or
a similar illness. As a thank you for taking the survey, you will receive
a $25 gift card.

After you take the survey, you will receive health information. This information will be personalized to your specific health interests and
needs. Smokers who would like help quitting will also have access to
free nicotine patches or Zyban, with their doctor’s permission.
Some participants will be chosen at random to receive this health information through a new website, and some will receive the health information in print form.

Both the website and the print materials will offer personalized
support for making health changes. The website and printed health materials are unique resources for patients who have had cancer, leukemia, tumors, or similar illnesses as adolescents and young adults. They cover issues and concerns that many survivors face, and
provide trusted information about important health topics.

If you choose to participate, you will be asked to take a short survey about 6 months from now. It will be emailed to you and will only take
about five minutes.

You will also be asked to take a final phone survey 15 months from
now. This one is similar to the first 20-minute survey, but will include questions about how well you liked the research project. You will
receive another $25 gift card.

Remember, your participation is completely voluntary and the Ulman Cancer Fund will not know if you chose to enroll. Your decision to join or not join this project will in no way impact your ability to receive services from or participate with the Ulman Cancer Fund.
