include your answers to the questions - ERASE EVERYTHING ELSE, including the questions (makes it much easier and quicker for us to
read, sometimes we get a lot of responses)

we need people for an upcoming study and we always have a new
one (all year long) - so please tell anyone you refer to reply to us
ASAP - answering all the requested information
If you don't hear from us within the next week or so it might just mean
we found all the people we needed and/ or you didn't qualify at this
time, but we'll still keep you in mind for other future studies.

If you are interested, please email us with all the information requested above and IF you might qualify for the current study we will call you
back to ask a few preliminary questions and then schedule you for a time to come in. We look forward to talking with you and hopefully having you participate.

Our studies range from all different types of products (which you may be familiar with) ie. - electronics/ computers/ high tech items, web sites, food items, automobiles, retail stores, TV & movies, sports, exercising, advertising, etc.

1) Name, PHONE #(s), Age, Gender, Occupation? FT/PT ? (or homemaker, student, unemployed, etc)

2) Do you own a cell phone and a computer (MAC or PC) - a digital camera, IPOD, PDA/ Blackberry/ Palm Pilot, etc ?? if you have a cell phone or PDA (etc) - what service company are you with? who is the manufacturer? do you have a single phone plan or family plan, etc ? WHO manages/ pays for your cell phone ?

3) What do you normally use your cell phone or PDA (etc) for - ie. just for calls or other uses like text messaging, taking/ sending pics or video, internet, listening to music or other cool features?? - please elaborate - - have you ever watched TV on your cell phone ?

4) Do you consider yourself a techno-addict and ALWAYS using or playing with your cell phone or other technology? - please elaborate

5) How do you typically pay your bills? online, in the mail, in person ??
which bills do you pay online? (ie. car, insurance, cell phone, mortgage, etc)
do you pay the bills in your household? if so what specific bills are you responsible for paying?

6) How often do you use the internet? do you use a computer/ internet at home or elsewhere?
Do you ever view and/ or pay your bills online? if not, are you interested or open to doing so?

The type of questions we primarily ask are opinion oriented, there are no right or wrong answers with our studies. The purpose of these types of studies is to help develop new products and services.
